Services provided to individuals and their families
We firmly believe in our clients’ ability to participate in their caregiving decisions. We rely on our experience and professional judgment to act in the best interests of our clients and to respect and consider their abilities to direct their own care in making safe caregiving decisions. We create a tailored care plan that includes recommendations regarding medical and health care issues, senior community resources, residence options, financial and legal planning, and more.
We are not affiliated with any other company or organization, and referral fees are never accepted or paid by Pathfinder Care Management/“Rent A Daughter™”.
Families often need precise information as well as reassurance. We help in the following ways:
- Liaison. We are the “eyes and ears” for family members, providing timely information that allows them to be involved when they can’t be present.
- Coordination of communications. We advocate with doctors and service providers to ensure understanding and approval of the treatment and approach provided.
- On-going support. We monitor services and provide regular updates to designated parties about changes and concerns.
- Moderate. We facilitate family meetings regarding care, enabling families to reach consensus and caregiving solutions, even when family dynamics are difficult or strained.
- Education. We provide ongoing, formal and informal education to families on the changes and challenges associated with aging and age-related disease processes.

Services provided to businesses
We serve as a resource for the community on aging. We provide the following services to other professionals who work with seniors:
Central point of communication. We work with doctors, counselors, home health aides, and other professionals to ensure thorough communication for a client’s caregivers.
“In-service” education sessions. We frequently speak to area assisted living facilities, private banking groups, and others to educate them about senior-specific concerns and issues. We have given CEU-credit courses on topics such as Home Care Options and Senior Depression.
Public speaking. Our staff have been featured speakers for the Alzheimer’s Association, Minneapolis Area Senior Workers Association, and the Age Odyssey Conference.
