Our Process
Initial Consultation
The initial consultation is a brief interview that is conducted to determine the appropriateness of our services for your needs. Our care manager will review the services that Pathfinder Care Management/Rent A Daughter™ provides and can arrange, discuss our service agreement and fees, and answer any questions you may have. There is no obligation.
Assessment
Once you decide to retain Pathfinder Care Management/Rent A Daughter™, a Care Manager will conduct a face-to-face interview, preferably in the senior’s place of residence, to review the following:
- Functional status: ability to perform activities of daily living (ADL’s), such as dressing, grooming, walking and transferring, and incidental activities of daily living (IADL’s), such as grocery shopping, making telephone calls, paying bills.
- Psychosocial status: support systems, past occupations and roles, values and preferences, spirituality, use of leisure time, recreational activities.
- Health status: chronic and active illnesses, medications, use of medical and assistive devices, sensory impairments, dental status.
- Legal and financial status: current health care coverage, ability to fund their own care and services, documentation of future wishes, wills and living wills/advanced directives.
- Cognitive status: ability to make decisions and communicate, mood or behavior issues, ability to exercise good judgment in an emergency.
- Nutritional status: appetite and intake, weight changes, food preferences, special diets, eating patterns, chewing or swallowing problems.
- Environmental issues: location, general condition and appropriateness of housing, accessibility, safety.
Plan of Care
The Care Manager will use the information obtained from the assessment to develop an individualized, written plan of care. This document will identify the strengths, needs goals, and recommended interventions. The interventions may include referral to services, home adaptation suggestions, and information regarding resources. The care plan can be revised and modified as the goals are reached or if new challenges arise.
Implementation and Coordination
After reviewing the care plan with you, we work with you to implement the interventions that have been identified. This step in our process is the action plan and varies in terms of length of time and type of work done, depending on your needs. We may research and arrange services, facilitate communication between family members, accompany the senior to doctor visits, or assist with submission of medical bills. The care manager will only initiate services detailed in the care plan or requested by you.
Monitoring and Ongoing Involvement
Our care manager will periodically monitor the efforts carried out to ensure that they continue to be appropriate and cost-effective. We will also reassess your plan of care and revise it as needed.
